Nominator Information
Please describe the act or acts of heroism for which the nominee displayed personal bravery or self-sacrifice so extraordinary as to set the individual apart from his or her fellow emergency service providers whereas they risked their personal safety of life.
Please describe the outstanding contributions that the nominee has made to the welfare of the fire, rescue and emergency medical services of Maryland. In addition please describe distinguished services that the nominee has provided to their department, other departments; and to their county, other counties, regional organizations and the State of Maryland.
Please describe how the nominee has emphasized leadership, innovation, professional development, safety and health, integrity, service to the public and contributions to the fire and emergency services at the local, state and national levels.
Please describe how the nominee has provided distinguished service to the MFCA and an individual department or county fire or rescue service.
Nominees must have a minimum of ten (10) years of service to be eligible for consideration.
Nomination to the Hall of Fame should be based upon distinguished service to the MFCA. Distinguished service to the nominees’ individual department/company or county may be considered.
